FOR PROPERTY,  LIABILITY & EQUIPMENT BREAKDOWN CLAIMS:

1. Report by telephone to the Office of Facilities & Risk Management: (618) 233-1090;  FAX: (618) 233-1696
2. Property & Equipment Losses - Protect Property from further damage; arrange for temporary repairs, if required. Property Damage Report (PDF)
3. Burglary/Theft Losses - Report incident to police or sheriff promptly.
4. Accidents/Injuries on Premises - Complete Accident Report Form (used for reporting of non-work related injuries)  and mail or fax to Office of Facilities & Risk Management.  Do not agree to pay any medical bills or other expenses.
5. Lawsuit Papers/Letters from Attorneys - Contact Facilities/RM Office by telephone on day received (618-233-1090); forward by registered mail to Office of Facilities & Risk Management.

FOR AUTO CLAIMS:

Report accident directly to Church Mutual Insurance 800-554-2642 (option 2) by telephone as soon as possible

FOR WORKER'S COMPENSATION CLAIMS:

1. Report by telephone Church Mutual Insurance Company:

2. Complete Form #45 (Employers First Report of Injury) and fax to Church Mutual within three working days of the accident.

FOR VOLUNTEER ACCIDENT CLAIMS:

The Diocese procures a Volunteer Accident policy issued by Nationwide Life Insurance Company for volunteers who are injured while in the process of a volunteer activity on behalf of a Parish or the Diocese.  The Volunteer Accident policy continues to provide a maximum benefit amount up to $25,000 payable over a three-year benefit period.  Coverage applies for eligible expenses that are in excess of amounts payable by any other health care plan.
Claims are administered by K & K. In the event of a claim, the incident report and accident medical insurance claim forms needs to be submitted at the same time to prevent a delay in the process of a volunteer claim. Forms can be obtained through the office of the Risk Manager.
